The 36-Hour “Super Yogurt” Taking Over Your Feed, Fact-Checked

Ten crushed probiotic tablets, a quart of half-and-half, 36 warm hours — and claims running from flatter stomachs to more empathy. The organism is genuinely interesting. The résumé built on top of it needs an audit, so here is one.

L. reuteri yogurt is a home ferment popularised by cardiologist William Davis: milk fermented with two L. reuteri strains (DSM 17938 and ATCC PTA 6475, originally from crushed BioGaia Gastrus tablets) plus inulin at around 100°F for 36 hours. Legally it is not yogurt — the FDA definition requires two bacteria this ferment does not contain. The strains have real but narrow human evidence: DSM 17938 reduced crying in colicky breastfed infants by about 25 minutes a day in a Pediatrics meta-analysis, and ATCC PTA 6475 roughly halved tibia bone-density loss in a year-long trial of 90 women aged 75-80 at 10 billion CFU per day. The recipe looks like nothing: a quart of half-and-half, a spoonful of inulin, and ten crushed tablets of an over-the-counter probiotic, held warm for 36 hours until it sets. The result — thick, tangy, sold to you by your feed as “the yogurt that changes everything” — is the most interesting food trend in gut health right now, and the most confidently oversold. The claims range from flatter stomachs to thicker skin, deeper sleep, more muscle and, remarkably often, more empathy. The organism at the centre of it is genuinely worth knowing about. The pitch built on top of it needs a fact-check, so here is one.

The trendWhat people are actually making

The protocol comes from cardiologist William Davis — the Wheat Belly author — via his book Super Gut, and his canonical recipe is specific: the bacterium Lactobacillus reuteri (two strains, DSM 17938 and ATCC PTA 6475), fermented in dairy with inulin at around 100°F for 36 hours — four times longer and noticeably cooler than regular yogurt. The original starter is BioGaia Gastrus, a chewable tablet whose label suggests one tablet a day for occasional gas and bloating and says nothing about fermentation — the yogurt is an off-label hack of a supplement. Two more facts belong in the picture. Legally, the result is not yogurt at all — the FDA’s definition requires two specific bacteria this ferment does not contain, a point Davis himself concedes. And commercially, the inventor of the trend now sells his own starter capsules, which he recommends over the tablets he built the recipe on. None of that makes the yogurt bad. All of it is context the videos skip.

The evidenceWhat this bacterium has actually done in humans

L. reuteri has real human research — unusually good research, for a probiotic. It is just about different things than the trend claims. The flagship is infant colic: for the DSM 17938 strain, an individual-participant meta-analysis in Pediatrics found it cut crying time in colicky breastfed babies by about 25 minutes a day — a real effect, in a specific population, at drop-sized doses. The most interesting adult result belongs to the other strain: in a year-long randomised trial of 90 women aged 75 to 80, ATCC PTA 6475 roughly halved the rate of shin-bone density loss versus placebo. That is a genuinely striking finding — and note the dose: ten billion CFU a day. Hold that number.

Now the claims your feed actually makes — oxytocin, connection, skin, muscle, libido. Those trace back to a series of striking studies in which L. reuteri sped wound healing, restored social behaviour and preserved testosterone… in mice. Largely one lab’s mice. And where the mouse findings have been taken into humans, the record is sobering: a 12-week randomised placebo-controlled trial of the exact strain in older men found no effect on testosterone at any dose — the one glamour claim that has been directly tested is the one that failed. The closest thing to human social-behaviour data is a small pilot trial of both strains in autistic children, which reported improved social-functioning scores but no change in anything else it measured, including the microbiome itself. No human study has ever shown these strains raise oxytocin in a person. The empathy yogurt is, for now, an empathy mouse.

The one glamour claim that has been directly tested in humans — testosterone — failed. The rest have not been tested at all.

The counting problemAbout those 300 billion CFU

The trend’s most repeated number is that a half-cup serving carries 200 to 300 billion CFU — a dose that would dwarf commercial probiotics. Follow the citation and it leads to Davis’s own blog: a flow-cytometry count from an unnamed lab, with no stated distinction between live bacteria and dead ones — and flow cytometry counts both unless you specifically ask it not to. No independent, published count of the finished ferment exists. Community lab tests organised by hobbyists — unverified, but the only outside data anyone has — have suggested the opposite problem: that after 36 warm hours, much of what is growing in the jar may not be L. reuteri at all. And here is the sleight of scale worth noticing even if Davis’s count is exactly right: the bone trial used ten billion a day. The mice behind the wound-healing claims got a few hundred thousand organisms. Nothing anywhere shows that 300 billion does more — the claimed dose is biggest precisely where the evidence is smallest.

The kitchen partA 36-hour ferment is a commitment, and the details are the safety

Fermenting dairy at body temperature for a day and a half is a bigger ask than the videos let on, and the people most invested in the trend say so themselves: sterilise everything, retire a culture that changes smell or texture, accept that some batches separate or fail. The window matters because 100°F is a temperature every microbe enjoys, not just the one you invited; the ferment is safe when acidification wins the race, and Davis’s own numbers — a finished pH around 3.5, plus reuterin, an antimicrobial this species genuinely produces — describe a well-protected end product when the ferment takes. A stalled batch is not a stronger batch; it is a warm jar of dairy that never got its defences up, and it belongs in the bin, not in a bowl with granola.

Two groups have specific homework beyond kitchen hygiene. This species is a documented histamine producer — the 6475 strain carries the genes and converts dietary histidine to histamine, and a 36-hour ferment is a long accumulation window — which is worth knowing for anyone who reacts to aged and fermented foods generally. And the NIH’s probiotic fact sheet notes that the rare serious probiotic infections on record have overwhelmingly involved people who were severely ill or immunocompromised; for anyone in that category, a home-brewed high-dose ferment is a conversation with their clinician first, not a food trend. For generally healthy people, decades of L. reuteri use — it has lived in human microbiomes as long as there have been humans — suggest the ingredient itself is not the risk; the process is where the care goes.

The kernelWhy this trend is more interesting than most

Strip the overclaims and something genuinely worth keeping remains. L. reuteri is measurably disappearing from industrialised microbiomes — it was a routine human resident two generations ago and is now rare in Western guts, while remaining far more common in non-industrialised populations. A food trend built around re-culturing a lost human commensal is a better idea than most of what goes viral in this space; compare it with the parasite-cleanse economy and it is practically scholarship. The honest summary: a real organism with real but narrow human evidence — colic in breastfed infants, bone density in elderly women, some GI findings — wrapped in claims imported from mouse studies, carrying a headline dose nobody has independently verified and no evidence anyone needs. If you enjoy making it and you ferment carefully, you are eating a tangy, high-dose probiotic ferment and that is a perfectly good thing to be doing with a Tuesday. Just let the yogurt be food — it has not earned the rest of the résumé yet. Does the yogurt really contain 200–300 billion CFU per serving?
That figure is self-reported by the protocol's inventor, based on flow cytometry from an unnamed lab, without a stated live-versus-dead distinction — and flow cytometry counts dead cells unless viability staining is used. No independently published count of the finished ferment exists. Community-organised lab tests (unverified, but the only outside data available) have suggested a large share of what grows over the 36 hours may not be L. reuteri at all. It is also worth asking what the number would buy: the best human trial of the strain used 10 billion CFU a day, and no study shows higher doses do more.
Is L. reuteri yogurt safe to make at home?
For generally healthy people who ferment carefully, the ingredient itself has a long history in humans and the finished ferment is acidic enough (around pH 3.5, by the inventor's own measurements) to be well protected — when the ferment takes. The care points are real, though: 36 hours at body temperature favours whatever microbes are present, so sterilised equipment matters, and a batch that smells wrong, never thickens or separates strangely should be discarded rather than rescued. Why is L. reuteri called a "lost" gut bacterium?
Because the disappearance is measurable. Microbiome researchers have documented that L. reuteri was a common resident of human guts in the mid-20th century and is now rare in industrialised populations, while remaining substantially more common in non-industrialised ones — a pattern attributed to diet, antibiotics and lifestyle rather than chance.
